Change of Address Form

Originated by the United States Postal Service, a change of address form is a form that informs USPS that an individual is moving from one primary residence to another. There are two types of moves: “Permanent” with signifies a one-way move, or a “Temporary” move with the intention of returning to a residence at a later date (lasting up to one year). Change of address forms are signified by a start date to let all correspondence know that a resident no longer lives there from a set date forward.
